Evaluation: Data Entry Cover Sheets

These documents serve as the cover sheets for local SRTS programs to use when sending their completed Student Tallies and/or Parent Surveys to the National Center for processing. They collect the information needed to properly store data from the forms in an online database and provide access to that data for the local program.
