Please note: These Parent Survey questionnaires are offered in addition to the English and Spanish versions of the survey. The surveys below were provided to the National Center for Safe Routes to School by local SRTS programs across the country. The National Center cannot verify the accuracy of translation or the completeness of the "Other Language" questionnaires.
Any completed questionnaires using the language versions below must be entered directly into the data system by the local program. They should not be mailed to the National Center.
A: After you have collected your non-English or non-Spanish Parent Surveys, you can enter them into the online data system by using the instructions at General instructions for using the SRTS online data collection and reporting system (PDF).
A: Yes, you can still send your English or Spanish Parent Surveys to the National Center for processing.
For mailing your Surveys to the National Center see instructions for creating and printing cover sheets.
* Please remember that if you have English, Spanish and Chinese (or other language) surveys for one school, enter the Chinese surveys into the same Parent Survey Set in which you want your English or Spanish surveys to live once the National Center processes them. A Parent Survey Set is the place holder for the individual Parent Surveys that are collected for a specific school at a specific time period (ex. Nov 2010). This will ensure that all your surveys for a particular school and time will be in the same set and will be accounted for in your Parent Survey summary report.
A: Email a copy of your survey to data@saferoutesinfo.org. Include a copy of the survey and who should be credited with providing the translated copy.
